Cardiomyopathy

Cardiomyopathy is an inflammatory disorder of the heart muscle. There are three types of cardiomyopathy—dilated, hypertrophic, and restrictive. The type found in celiac disease is the most common form, dilated (congestive) cardiomyopathy. In this condition, the heart muscle becomes inflamed, weakens and, as a result, the heart dilates (enlarges). It cannot pump normally, and most patients develop heart failure. Patients also have arrythmias (disturbances in the heart’s electrical conduction) that may cause palpitations and further impair cardiac function.

There is evidence that the autoimmune reaction against the heart is directed against tissue transglutaminase (tTG) present in the heart. In patients with an associated autoimmune cardiomyopathy, treatment of the celiac disease with a gluten-free diet has been shown to improve cardiac function.
